Please print the New Student Health Services Checklist. and use it to ensure you have completed your Health Services requirements prior to arrival at ºÚÁϳԹÏÍø±¬ÍøÕ¾.

• All ºÚÁϳԹÏÍø±¬ÍøÕ¾ students must comply with  as well as requirements put in place by the College. 
• ºÚÁϳԹÏÍø±¬ÍøÕ¾ requires and recommends the following vaccinations. See Immunization Form. You may submit this form OR an immunization report from your healthcare provider (HCP).
• Obtain and review your immunization record from your healthcare provider (HCP) ASAP. Many students can access their records through their health care portal.  Do not wait until you have an appointment to do this.
• Make an appointment to receive any required immunizations you are missing over the summer.
• Submit your immunization record via the  as soon as possible. If you receive immunizations over the summer, you should upload documentation promptly.
• Delays in submitting your immunizations could lead to a delay in access to your room upon arrival.

Print your New Student Health Documents

• Physical Exam Form
  • All students must submit documentation of a physical.
  • This form is strongly recommended for new students and required for all athletes.
  • Students not participating in athletics may submit a physical performed within the last 18 months.

• Immunization Form
• • This form lists ºÚÁϳԹÏÍø±¬ÍøÕ¾’s required and recommended immunizations.
  • We will also accept records printed from your HCP’s patient portal.  Records must be in English.
  • If you are missing any required immunizations, please plan to receive them prior to arrival on campus.
  • Ideally, your immunizations are complete upon arrival.  If not, you will need to make a plan with Health Services to receive any immunizations that are unobtainable at home.
  • Students without evidence of immunization or documented disease must submit blood test results that show immunity against varicella, measles, mumps, rubella, and hepatitis B. 
• TB Form(s)
  • Students answering YES on the Tuberculosis Screening Questionnaire on the student health portal must have their HCP complete Tuberculosis Testing Form.

Register for the student health portal using your username@bowdoin.edu address. Enter your password for your ºÚÁϳԹÏÍø±¬ÍøÕ¾ account.

• In the student health portal select the My Forms tab.
• Complete the Medical History and Wellness Form.
• Complete the Immunization History Form using the immunization documentation obtained from your HCP.
• Read and sign the Consent Form.
  • Must be signed by the student.
  • If you will be under 18 when you arrive on campus, your parent or guardian must sign.
• Complete the Tuberculosis Screening Questionnaire.
• Complete the HS Plan Information Form.
• The Physical Form PDF and Immunization Form PDF can also be printed from the My Forms page.

Documents with the following file formats can be uploaded: JPEG, TIF, PNG, PDF, BMP, and GIF. Each file should not exceed 6MB.

Please upload the following documents via the upload tab embedded in your Portal Forms. ​​

• Immunization Documentation
  • Immunization documentation can be either:
    • A ºÚÁϳԹÏÍø±¬ÍøÕ¾ Immunization Form completed and signed by HCP OR
    • Immunization records from your HCP/Health Care Portal/Immunization Registry
  • Upload within the Immunization History Form after entering your dates of immunizations. ​
• ​Physical Exam Form
  • Completed and signed by HCP
  • Athletes must submit the ºÚÁϳԹÏÍø±¬ÍøÕ¾ Physical Exam Form. Please refer to the Athletic Training page () for further guidance.
  • Upload within the HS Physical Form and submit. ​
• ​TB Form(s) — only if indicated by screening
  • If indicated by your TB Screening Form, the TB Testing Form must be printed and completed by your HCP.
  • Upload within the TB Screening Form and submit. ​​
• Insurance Cards
  • If prompted by HS Plan Information Form, take photos of the front and back of card.
  • Upload images within HS Plan Information Form.
  • If insurance changes during the school year, please upload a new card.
